Appointment information

Family medicine & ongoing care

Ongoing care for accepted patients, including chronic-condition follow-up, mental health concerns and coordination of care.

Discuss new symptoms, review longer-term conditions such as diabetes or high blood pressure, and plan follow-up with your physician. Mental health appointments can include assessment, treatment planning and appropriate referrals.

Appointment type
In person, with follow-up by telehealth when appropriate.
Before your visit
Bring your medication list, relevant reports and the main concerns you would like to discuss.

Mental health care

Assessment and follow-up for mental health concerns, with treatment planning and referrals when appropriate.

Contact the clinic from November 2 to discuss availability and the appointment you need.

Appointment type
In person or by telehealth when appropriate.
Before your visit
Tell the team what you need when arranging your visit.

Medication reviews & renewals

Review your medicines, discuss side effects and assess prescription renewal requests.

Your clinician will review whether a prescription or change is appropriate. An examination, monitoring or further information may be needed before a medicine can be prescribed or renewed.

Appointment type
In person or by telehealth, depending on the assessment needed.
Before your visit
Have the medication names, doses and pharmacy details ready. Arrange a review before your supply runs low; administrative email is not a prescription service.

Preventive care & screening

Personalised health reviews, women’s health concerns and discussion of appropriate screening, including cervical screening.

Your clinician can review your history, risk factors and screening needs, and discuss which tests or next steps are appropriate for you.

Appointment type
An assessment, with in-person appointments for examinations or sample collection when needed.
Before your visit
Bring any screening invitations, previous results and relevant vaccination records. Confirm the appointment type when booking.

Telehealth

Phone or video appointments for concerns and follow-up that can be assessed appropriately at a distance.

A clinician will determine whether virtual care is suitable. An in-person visit may be needed for a physical examination, a procedure or a concern that cannot be assessed adequately remotely.

Appointment type
Phone or video, as arranged by the clinic.
Before your visit
Choose a private place, have your medication list ready and make sure the clinic has your current contact number. Follow the instructions provided for your appointment.

Test requests & referrals

Assessment of the need for laboratory tests, imaging and referrals to other health professionals.

Your clinician can arrange appropriate requisitions or referrals following an assessment. Laboratory tests and imaging are carried out by the relevant testing provider. Referral appointments are arranged through the receiving service.

Appointment type
A primary care appointment, with testing or referred care arranged separately when indicated.
Before your visit
Bring relevant reports and any requisitions you have received. Follow preparation and booking instructions from the testing provider.

Arrange your visit

Check appointment information and confirm the service you need before booking. Some examinations, forms and treatments may have a fee; ask about any applicable charge in advance.
